For schools and teachers

Digital Permission Slips and Forms for Schools

Send one field trip slip to every parent in the class by text or email. They sign on their phone in a minute, and you see who has signed and who hasn't, without chasing paper in backpacks.

Documents you can send

The forms that come home every week.

Upload your school's own form as a PDF or a photo. Add the fields parents need to fill in.

  • Field trip permission slips, with the date, the cost and an emergency phone number.
  • Photo and video releases, with a checkbox for each use you are asking about.
  • Sports and activity forms, including the participation waiver and emergency contacts.
  • After-school and club sign-ups, so you know who is coming before the first day.
  • Acknowledgements, like the student handbook, device use policy or a change in pick-up times.
  • Forms that need a document back, such as a sports physical: the parent attaches a photo or PDF with an "ask for a document" field.

How it works

One slip, the whole class, three steps.

1. Upload the slip

Upload the PDF or snap a photo of the paper form. Mark where the parent signs, and add fields for the student's name, a phone number or a checkbox.

2. Send a copy to each parent

Paste the list of parent emails or mobile numbers. Each family gets their own copy by text or email.

3. Watch them come back

The status page shows who has signed. Parents who haven't get email reminders every two days, up to three times.

Why MultiDocs

Made for parents who are busy and teachers who are busier.

No account for parents

Parents open a link and sign with a finger or a mouse. Nothing to download, no password to remember.

Text, WhatsApp or email

Send the link by text message, WhatsApp or email, or copy it and paste it anywhere. Email reminders go out every two days, up to three times.

Bulk send

A copy for each parent: up to 20 families on the free plan (it counts as one request), enough for most classes. The Team plan sends to up to 500 at once, for a whole grade or school.

Audit trail on every PDF

The last page records the date and time, where the link was sent, IP address, device, a unique signer code and the SHA-256 fingerprint of the file. Anyone can check it at /verify.

Good to know

What we collect, and when we delete it.

Student information is sensitive, so here are the facts. MultiDocs does not claim to be FERPA compliant, and we don't sign data agreements as a "school official." Your school or district decides whether a tool fits its policies, so check with your district before you send forms that contain student records. The U.S. Department of Education explains the rules at studentprivacy.ed.gov.

What we store: the document you upload, the names, emails and phone numbers you enter, what parents type into the fields, and for each signature the date and time, IP address, device and browser. Files are encrypted in transit, opened only through a personal link, and deleted 30 days after the document is completed. We don't sell data or show ads. Download the signed slips and keep them in your school's own records.

A simple rule that helps: only ask for what the trip or activity really needs. A permission slip rarely needs a date of birth, a grade or medical details beyond an emergency contact.

Questions

Questions from teachers and front offices.

Can I send a permission slip to every parent at once?

Yes. Bulk send gives each parent their own copy. On the free plan you can send to up to 20 people at once and it counts as one request; on the Team plan, up to 500.

Do parents need to download an app or create an account?

No. They get a link by text or email, open it in their phone browser and sign with a finger. Nothing to install.

Can both parents sign the same form?

Yes. A document can have up to 20 signers, at the same time or in order, so you can ask for both parents or guardians when your school requires it.

Is MultiDocs FERPA compliant?

We don't claim FERPA compliance. We tell you exactly what we collect and that files are deleted 30 days after completion, so your school or district can decide. Avoid putting education records in forms unless your district has approved the tool.

Is an e-signed permission slip valid?

In general, yes. Under the federal ESIGN Act and state UETA laws, a signature can't be refused just because it is electronic. Your district may still have its own rules about which forms must be on paper.

How much does it cost a teacher?

Nothing for one class: the free plan includes 15 requests a month and bulk send to 20. A whole school can use the Team plan for $25 a month, with unlimited staff users.

Pricing

Free to start. $25 a month for the whole team.

Sign your own documents free, forever, and send 15 signature requests a month on the free plan. The Team plan is $25 a month (or $240 a year) for everyone in your school, with up to 1,000 requests a month and bulk send to 500 people.
