Good to know

What the form can and can't do.

Age and ID. Tattoo and piercing rules for minors are set by each state, and many require a parent present in person or ban tattoos for minors entirely. A client can attach a photo of their ID, but MultiDocs doesn't verify identity or age. Check the ID in person, as your state requires.

Health questions. MultiDocs is not HIPAA compliant and doesn't sign business associate agreements. Most salons and tattoo shops aren't HIPAA covered entities, but a medical spa working with a physician may be. If that's you, use a HIPAA tool for medical forms.

Signed forms are deleted from MultiDocs 30 days after completion, so download them and keep them as long as your state health department or insurer requires. This is general information, not legal advice. For your specific situation, ask a lawyer licensed in your state.

Questions

Salon and tattoo studio questions.

Can clients sign a tattoo consent form on their phone?

Yes. Text them the link with the booking. They open it, fill in the fields and sign with a finger. No app and no account.

Is a digitally signed consent form valid?

Under the ESIGN Act and state UETA laws, an electronic signature can’t be denied legal effect solely because it is electronic. Your state may have its own rules about what a tattoo consent form must include.

Can the client upload a photo of their ID?

Yes, with an "ask for a document" field. MultiDocs does not verify the ID, so still check it in person.

Can every artist in the shop use it?

Yes. The Team plan is $25 a month with unlimited users, so each artist can send their own forms from one shared dashboard.

Is MultiDocs HIPAA compliant?

No. We don't offer HIPAA compliance or BAAs. Don't use MultiDocs for forms that a HIPAA covered entity must protect.

What does it cost?

The free plan includes 15 signature requests a month. Busy shops use the Team plan: $25 a month or $240 a year, up to 1,000 requests.
