Free Tattoo Consent Form Template (Clients Sign from Their Phone)
A tattoo consent form is the document a client signs before getting tattooed. It confirms they are of legal age, that they are healthy enough and sober, that they approved the design and placement, that they understand the risks, and that they received aftercare instructions. Most state and local health departments require studios to keep a signed form for every client. This template covers all of it, and clients can sign it from their phone before they walk in.
In 30 seconds
- A tattoo consent form records the client's ID and age, health questions, approval of design and placement, the risks, aftercare and a release.
- 18 is the usual minimum age. Some states allow minors with a parent's consent and presence, some ban tattooing minors entirely. Check your state.
- The practice of tattooing is regulated by state and local authorities, which often set what the form must include and how long to keep it.
- Keep health answers private and only ask what you need for safety.
What a tattoo consent form should include
- Client identity and age: name, date of birth, contact details and the type of photo ID you checked.
- Health questions: conditions that affect healing or safety, such as diabetes, bleeding disorders, blood thinners, skin conditions in the area, allergies to latex, metals or pigments, pregnancy, and recent alcohol or drug use.
- Design and placement: description, size, location and colors, approved by the client.
- Risks: infection, allergic reaction, scarring, color variation, the permanence of the tattoo and the difficulty of removal.
- Aftercare: confirmation that written aftercare instructions were given.
- Release and a statement that the information is true.

How to have clients sign it before the appointment
- Upload the form. Add your studio name and license, save it as a PDF and upload it to MultiDocs. Upload once and reuse it.
- Place the fields. Text fields for the client's details and design, checkboxes for the health questions and photo choice, initials next to each statement, a signature and date, and an "ask for a document" field so the client attaches a photo of their ID.
- Send it when you confirm the booking. By text message, WhatsApp, email or a link in your booking confirmation. Clients sign from their phone without an account. Running a flash day or a convention? Bulk send gives each booked client their own copy.
- Get it back with the audit trail. Before the client sits down you have the signed form, their ID photo and a certificate showing the date, time, device and the phone or email the link went to. Check the ID against the client in person.
Age and state rules
There is no federal law on who can be tattooed or how studios must operate: as the FDA puts it, the practice of tattooing is regulated by local jurisdictions. That means state health departments, and often counties or cities, set the rules on licenses, sanitation, record keeping and consent forms.
- Minimum age: 18 is the general rule. Some states allow tattooing minors with written consent from a parent who is present; others ban it under 18 regardless of consent. Some set different rules for minors who are emancipated or for particular placements. Check your state statute and your local health code before you tattoo anyone under 18.
- What the form must say: some health codes require specific health questions, written aftercare instructions, or a notice about the risks. Add anything your jurisdiction requires.
- Record keeping: many jurisdictions require studios to keep consent forms for a set number of years. Download each signed PDF to your own records, since MultiDocs deletes files 30 days after completion.
The FDA regulates tattoo inks, issues recalls and warns that contaminated inks can cause infections even when the artist follows good hygiene. Recording ink brands and lot numbers in the "studio use only" box helps if there's ever a recall.
Health information and privacy
A consent form collects health details, so treat it with care: ask only what you need for safety, keep forms where only authorized staff can see them, and don't share them. Most tattoo studios are not HIPAA covered entities, but some state privacy and health-record laws may still apply. Note that MultiDocs is not a HIPAA-compliant service and does not sign business associate agreements, so if your business is a HIPAA covered entity, such as a medical clinic offering cosmetic tattooing, don't use it for protected health information.
This is general information, not legal advice. For your specific situation, ask a lawyer licensed in your state.
FAQs
Do tattoo studios need a consent form?
In most places, yes. Many state and local health codes require a signed consent form for each client, kept for a set period. Even where it isn't required, it is the studio's main record.
What age do you have to be to get a tattoo?
Generally 18. Some states allow minors with parental consent and presence, others don't allow it at all. Always check the ID in person.
Can a client sign a tattoo consent form electronically?
Yes. Under the ESIGN Act and state law, an e-signature is valid. If your local health code requires a specific format, upload that form instead of this template.
Can a parent sign for a minor to get a tattoo?
Only where state law allows it, and many states require the parent to be present and show proof of their relationship to the minor. In some states it isn't allowed at all.
Does a tattoo consent form protect the studio from lawsuits?
It helps show the client was informed and consented, but it won't protect against gross negligence, such as unsterile equipment, and how much a release protects you varies by state.
Should aftercare instructions be on the consent form?
Give them as a separate sheet the client keeps, and have the client confirm on the form that they received them, as this template does.
